Generally looking pretty good. I think you could have a bit smoother lift off, it seems like you have one speed off the floor and the jolt of that pull is flattening your back (stripper pull). Try for a smoother, even slower, lift off and then accelerate rapidly once you past the knees. Your numbers might dip a bit while you get used to it, but I think you'll be better off in the long run. Keep it up!
I really think the jerk looks pretty decent. One thing I like to tell everyone (its a cue from greg everett) is to aim for the back of your neck when you jerk. That helps me a lot. I think your depth is fine. On the miss at 125, you just didn't commit to getting under the bar. You had plenty of drive, but you need to work on changing directions. you could practice by doing a modified "snatch balance" with a very light weight, and instead of dropping into the snatch catch, drop into your jerk. But overall, I'd say you're doing really great! Keep it up!
